Participates in discharge planning for each resident. Participates in treatment planning, goal setting, and progress reviews for each resident. Works with other group home staff to help each resident to maximize achievement of program and treatment goals. Provides guidance and direction to residents in a manner that develops and nurtures personal values and a solid system of communication. Prepares breakfast and packs school lunches for residents in care. Maintains residents\u2019 files in compliance with agency procedures, and files all paperwork, including daily logs, group logs, incident reports, and related documentation. Serve as a role model for residents in the areas of ethics, morals, appearance, spirituality, and lifestyle principles. Transports kids to appointments\/school as necessary.\u00a0<b>Must have clean driving record. EVENING &amp; WEEKENDS!<\/b><\/p>\n<p>Education &amp; Experience<\/p>\n<p>Ability to teach and model independent living and social skills.
